Report FAMLI Fraud

Many states have seen a rise in fraudulent activity, identity theft, phishing attacks, and other scams targeting claimants of wage replacement programs like Unemployment Insurance and Family and Medical Leave Insurance. Below, you’ll find tips on how to protect yourself, as well as options to report identity theft, a compromised My FAMLI+ account, or other suspected fraud related to Colorado’s FAMLI program. 

For Individuals

You should submit a Report FAMLI Fraud form if:

  • You received a suspicious text message or email about your FAMLI claim.
  • You or someone else in your household has received a ReliaCard debit card or other correspondence but have not filed a FAMLI claim.
  • Your household has received a ReliaCard or other correspondence at your address, but in the name of someone who does not live in your household.
  • You believe your My FAMLI+ account or FAMLI claim has been compromised.

After reporting fraud, we recommend taking the following additional steps:

  • If you received a U.S. Bank ReliaCard debit card that you did not request, deactivate the card by following the instructions at usbankreliacard.com.
  • Contact the three consumer credit bureaus and put a fraud alert on your name and Social Security number (SSN).
  • Create a file where you can keep any records related to the fraud or identity theft in one place.

For Employers

When a FAMLI claim is filed by one of your employees, you will receive documentation from the FAMLI Division alerting you of the claim. The initial documentation will include the length of the leave requested and the employee’s reported work schedule. You’ll also receive subsequent documentation with the status of that employee’s claim.

Employers should submit a Report FAMLI Fraud form if:

  • No person with the name or SSN (or Individual Taxpayer Identification Number) included in the claim documentation sent by FAMLI works for you.
  • A person with the same SSN or ITIN works for you, but the name is different.
  • A person with the same name works for you, but the SSN or ITIN is different.
  • A person with the same name and SSN or ITIN works for you, but their reported work schedule is different.

Protect Yourself Against Fraud and Identity Theft

Here are some steps you can take to protect yourself against attempts to compromise your My FAMLI+ account and FAMLI claim:

 Be careful about requests by phone, text, or email to share personally identifiable information such as your Social Security Number or ITIN, bank account numbers, PIN numbers, or account passwords. Scammers try to obtain personal information in order to file fraudulent claims or compromise existing accounts.

 Avoid clicking links in suspicious emails and text messages. If you are unsure about any link asking you to log into your account, simply disregard the link and instead access your My FAMLI+ account from the official FAMLI website.

 If you have filed a FAMLI claim, regularly check that your address, phone number, email, and payment method are correct and have not been altered.

 Do not share your My FAMLI+ password with anyone.
